Description

This role involves providing comprehensive, value-based, medication-related care to improve patients' quality of life. The pharmacist collaborates with healthcare professionals to ensure optimal drug therapy outcomes. Responsibilities include ensuring medication safety and efficacy, processing and dispensing prescriptions, monitoring therapy, ensuring regulatory compliance, overseeing pharmacist extenders, maintaining medication security, and safeguarding patient confidentiality. The pharmacist also provides drug information support to medical staff, ensures continuity of care, and identifies potential challenges in common situations.

Minimum Requirements:

• Current Indiana pharmacist license or eligibility for licensure in Indiana
• Successful completion of an ASHP-accredited PGY1 and/or PGY2 pharmacy residency may be required
