Job summary

Carolinas Medical Center in Charlotte, NC seeks a licensed Physical Therapist to provide direct therapy services to patients seven days a week, including holidays. You will assess patients, develop comprehensive care plans, and implement exercise programs and education.

You will document evaluations and progress, coordinate with physicians and the care team, manage scheduling and discharge planning, and supervise PTA/ Rehab Tech while maintaining licensure standards.

Qualifications

• Bachelor's degree in Physical Therapy is required.
• State license to practice Physical Therapy is required.
• BLS certification is required by policy.

Responsibilities

• Provide direct therapy services to patients seven days a week, including holidays.
• Evaluate patients and establish appropriate patient care plans with modalities and education.
• Gather data from medical records, physician reports, patient and family for documentation.
• Document evaluations, tests, progress notes, discharge notes and related charges.
• Communicate progress to physicians and care team; coordinate scheduling and discharge planning.
• Provide supervision of PTAs and Rehab Tech/Aide as required by licensure.
