Responsibilities

• Provide various personal care and related paraprofessional services in accordance with an established plan of care.
• Assist patients in their homes with personal needs and comfort.
• Assist with clients’ daily activities.
• Communicate effectively with clients and family members.
• Assist with bathing, dressing, toileting and basic activities of daily living.
• Maintain strong sense of awareness.
