Job summary

Active Day is seeking a Personal Care Aide to support clients at home, assisting with daily living activities, health monitoring, and safe transport if needed. You will contribute to ongoing well-being and independence for seniors and adults with special needs.

You must be reliable, communicate clearly, and adapt to a flexible schedule while following health and safety guidelines. Prior experience is preferred, along with a valid CNA/HHA certification.

Qualifications

• High school diploma or equivalent.
• Minimum age 18 for non-driving roles; 21+ for driving roles.
• Current Home Health Aide and/or CNA certification preferred.
• For driving roles: valid driver’s license, reliable transportation, and proof of vehicle registration and insurance are required.
• Minimum one year full-time equivalent experience in a health care setting preferred.
• Prior experience working with seniors and/or adults with special needs preferred.
• Strong communication and interpersonal skills.
• Ability to multi-task and manage time effectively.
• Flexible schedule.
• Ability to lift up to 50 pounds and stand for extended periods.
• Background check clearance and any applicable state regulatory requirements.

Responsibilities

• Provides support and assistance for activities of daily living and personal care as needed (e.g. arrival and departure, eating, ambulation, transferring, therapies, restroom assistance).
• Provides home support and maintains a healthy environment (light cleaning, laundry, shopping, home safety).
• Prepare and serve meals and snacks and assist with feeding as needed.
• Observe members for changes in condition or unusual occurrences; escalate concerns as appropriate.
• Run errands for groceries, personal items, appointments as needed.
• Ensure safety by monitoring activities and maintaining a sanitary, clutter-free environment.
• Communicate positively and professionally with members, families, coworkers and community.
• Comply with company policies, safety and regulatory laws and standards.
• Other duties as assigned.

Job Summary

Personal Care Aides are responsible for providing, restoring, maintaining, and promoting the health status of the clients through home support, medical monitoring, client transportation services, and assistance with activities of daily living. • Ensure the safety of members by monitoring activities, keeping a sanitary, clutter-free environment.
• Communicate in a positive, courteous and professional manner with members and their families, co-workers, referral agencies, and the community at large.
• Comply with company policies and procedures, safety and regulatory laws and standards.
• Other duties and responsibilities as changed or assigned at any time.
