Qualifications

  • Must have a high school diploma or GED with college credits or supervisor experience.
  • Two years supervisory experience required; leadership skills essential.
  • Associate or Bachelor’s degree preferred; healthcare experience valued.
  • Experience with Medicare/Medicaid and home care operations expected.
  • Strong computer proficiency and documentation skills required.
  • Excellent organizational and multitasking abilities; strong communication is mandatory.
  • Bilingual fluency may be required based on location; Spanish/other languages a plus.
  • Professional verbal and written communication skills are essential.

Responsibilities

  • Maintain accurate client service records, plans of care, caregiver schedules, and required documentation.
  • Review and manage service authorizations and contract-specific requirements.
  • Support branch growth initiatives and operational planning efforts.
  • Ensure ongoing compliance with licensure, payer, contract, and regulatory standards.
  • Provide guidance, coaching, and support to caregivers to promote performance and engagement.
  • Conduct caregiver orientations as required.
  • Collaborate with payroll and timekeeping teams.
  • Monitor daily service delivery to ensure adequate caregiver availability and coverage.
  • Monitor systems to confirm authorizations align with scheduled services and utilization reports.
  • Communicate professionally with caregivers, clients, case managers, and leadership.
  • Represent Elara Caring in the community; perform additional duties as assigned.

Job Description: Care Team Supervisor, Personal Care Services Pay Per Visit | On-site | Must be available at least 3 days M-F 8-5 Tyler, TX

As the Care Team Supervisor, you'll contribute to our success in the following ways:
  • Maintain accurate client service records, plans of care, caregiver schedules, and required documentation
  • Review and manage service authorizations and contract-specific requirements
  • Support branch growth initiatives and operational planning efforts
  • Ensure ongoing compliance with all licensure, payer, contract, and regulatory standards
  • Provide guidance, coaching, and support to caregivers to promote performance and engagement

  • Perform additional duties and projects as assigned to support team and organizational success
What is Required?
  • High School Diploma or GED required
  • Sixty (60) college credit hours required an/or Two (2) years of supervisory experience required
  • Associate's or Bachelor’s degree preferred
  • Experience working with elderly or disabled populations preferred
  • Five (5) years of experience in a fast-paced office or healthcare environment preferred
  • Working knowledge of Medicare/Medicaid and home care operations preferred
  • Strong computer proficiency and documentation skills
  • Excellent organizational and multitasking abilities
  • Professional verbal and written communication skills
  • Strong leadership, coaching, and motivational skills
  • Ability to thrive in a fast-paced, high-volume environment
  • Bilingual fluency (Spanish, Mandarin/Cantonese, Russian, or Portuguese) may be required based on location
  • Reliable personal transportation and ability to travel locally and occasionally to support audits or operational needs
