Job summary

Mary Washington Healthcare is seeking an Hub Clinical Care Coordinator Assistant (Hub C3A) to provide non-clinical administrative support for the Hub and Care Coordination team. The role ensures timely admission and transfers, collaborating with the care team to improve throughput and efficiency.

The position supports documentation for patient transfers, inquiries from providers and families, and coordination of transportation and other logistics, while assisting with Medicare appeal processes

Qualifications

• High school diploma or equivalent required.
• Computer skills relevant to the position (Microsoft Office).
• Clear written and verbal communication abilities.
• Ability to collaborate as a team member.
• Ability to manage time and multiple priorities.

Responsibilities

• Presents a positive departmental and organizational image and adheres to policies and procedures.
• Processes documentation for patient transfers to post-acute settings (photocopies, faxes, electronic submission).
• Responds to inquiries from post-acute providers, adult protective services, patients, and families; directs them appropriately.
• Provides direct support for the Medicare Appeal Process.
• Monitors Care Coordination task list and coordinates patient transportation when needed.
• Communicates operational activities to leadership and participates in performance improvements.
