Job summary

Mary Washington Healthcare seeks a Unit Clinical Care Coordinator (Unit C3) to oversee unit-level patient progression, DRG and target LOS assignment, and discharge planning. You will lead a care team, coordinate with emergency and surgical services, and drive timely care through the Hub system.

The role requires strong nursing experience in acute and critical care, excellent communication, and ability to manage multiple priorities under pressure.

Qualifications

• Minimum of three (3) years recent acute care nursing experience.
• Minimum two (2) years of critical care experience.
• Bachelor of Science in Nursing is preferred.
• Able to work independently, managing time, multiple priorities, and resources to achieve goals.
• Able to maintain a calm demeanor and command during time of crisis management.
• Able to articulate information and ideas clearly through both written and verbal communication.
• Preferred: One (1) year supervisory experience strongly preferred Utilization review experience

Responsibilities

• Coordinates patient care activities with Clinical Care Coordinators to ensure appropriate level of care and patient flow.
• Identifies and removes unit barriers that impede patient flow.
• Assesses patients against MCG to identify admission status, working DRG and target LOS; validates orders and collaborates with CDI Nurse when needed.
• Assesses patients in observation/outpatient status to determine if status change is warranted.
• Coordinates with UR Nurse to align payor status with clinical progression and ensures regulatory letters are delivered.
• Educates medical and nursing staff regarding status, placement, payor requirements, and resource utilization.
• Develops discharge plan addressing patient needs, barriers, and readmission risk; consults PT, OT, Social Work as needed.
• Leads daily SNAP huddle to identify barriers and assigns resolution tasks; escalates unresolved barriers to leadership.
• Manages progression and discharge in Care Advance software and communicates status to Hub.
• Ensures plans of care are initiated in collaboration with clinicians and aligned with clinical pathways.
• Communicates plan of care and discharge plan to patient/family with the primary care team.
• Oversees preprocedural requirements and service diagnostics; communicates issues with care team.
• Acts as a resource on patient status and care coordination for unit Associates.
• Performs other duties as assigned
