CenterWell Senior Primary Care in Nashville, TN is seeking a Patient Specialist to ensure patients are prepared before the doctor sees them. This onsite, full-time role supports doctors by handling intake, documenting histories, and conducting validated screenings.

You will manage the EMR inbox, verify medications, and reinforce next steps using loop-back techniques to promote accurate patient understanding and health outcomes.

Qualifications

• Requires completion of MA training or equivalent; BLS certification may be required.
• Knowledge of HIPAA, infection prevention, OSHA and safety standards.
• Strong computer literacy and EMR proficiency.

Responsabilités

• Intake & clinical rooming including vitals, medication & allergy verification; patient history documentation; screenings as needed (PHQ-2, Fall Risk).
• Inbox & worklist management in EMR to route patient messages, lab results, and medication follow-ups; aim for same-day touch.
• Education & loop-back to ensure patient understanding and adherence to next steps.
