Job summary

CenterWell Home Health in Charlotte, NC seeks a Patient Services Coordinator to schedule visits, coordinate with field staff, patients and physicians, and support daily office operations.

You will manage patient schedules, handle staffing changes, and assist with case conferences, while ensuring timely communications and quality care. A minimum of 1 year in home health and strong organizational skills are required.

Qualifications

• Must have at least 1 year of home health experience.
• Excellent communication and prioritization skills.
• Coding certification preferred.
• Prior packet review / QI experience preferred.

Responsibilities

• Manages schedules for all patients.
• Edits schedule for agents calling in sick, ensuring patients are reassigned timely.
• Updates agent unavailability in worker console.
• Initiates infection control forms as needed, sends the completed “Employee Infection Report” to the HR/Department for upload.
• Serves as back up during lunch hour and busy times including field staff calls and weekly case conferences.
• Refers clinical questions to Branch Director as necessary.
• Maintains the client hospitalization log and shares updates with staff.
• Ensures staff are scheduled for skilled nurse/injection visits and add-ons as required.
• Schedules TIF OASIS collection visits and manages scheduling changes.
• Prepares weekly Agent Schedules and reviews productivity.
• Assists with transfer of patients between branch offices.
• May perform patient visits or participate in on-call rotation.
