Sentara Health is seeking a Remote Patient Service Coordinator to support Medicaid members and providers, working collaboratively to improve health outcomes. This role emphasizes effective information gathering and collaboration with the clinical team to implement interventions for better health and plan adherence.
The position also involves assisting with insurance verification, scheduling, and authorization processes to minimize denials and ensure smooth care delivery.
Patient Service Coordinator works in a collaborative role with Medicaid members and providers to improve health outcomes. Assists with gathering information for the health care team regarding barriers or other health information. Implements the interventions to promote improved health and compliance with recommended plan of care, as approved by the Health Care Team. This includes functions associated with the screening for changes in health status, coaching for improved self-management, reminders of necessary testing as determined by clinical guidelines, and support of the professional clinical staff in Health Care Services.
May serve as a key resource in supporting the clinical authorization processes, including insurance verification/obtaining pre-certification, scheduling, pre-registration and order transmission. Purpose is to ensure maximum reimbursement and reduce denials related to incomplete insurance information and/or authorization. Maintains statistics and acts as liaison between clinical testing sites and offices.
