Job summary

Sentara Health Plans in Winchester, VA is seeking a Care Coordinator to perform combined in-person and telephonic case management for members with complex health needs. The role requires collaboration with multidisciplinary teams and support for patients and families in navigating medical and social challenges.

The position is full-time (40 hours/week) with standard hours 8am-5pm EST, and travel up to 90 minutes one way may be required.

Qualifications

• Requires a degree in Health & Human Services or related field; Accredited institutions preferred.
• Experience with long-term care, Medicaid, and mental health settings is desirable.
• Strong collaboration with care teams and families is essential.

Responsibilities

• Perform casework duties and provide case management for patients, families, and designated caregivers.
• Develop and monitor multidisciplinary collaboration of services for patients.
• Assist adult patients and families with medical and environmental challenges, including terminal settings.

This position requires both in-person/face-to-face assessments and remote telephonic assessments for members with Serious Mental Illnesses, Serious Emotional Disturbances (ages 2+) and Substance Abuse Disorders!

Travel may include up to 90 minutes one way.

Job responsibilities

Performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses.

Education

• Must possess a degree in Health & Human Services or one of the following related Fields:

Certification/Licensure

Experience

• Long Term Care- 1 year (required)
• Health Plan- 1 year; Medicaid- 1 year (preferred)
• At least one year of mental health experience (preferred)
• 1 year of experience with Children/Adolescents (required)
