Job Summary

The primary function of the PSC is to schedule patient services and coordinate with staff. The PSC is also expected to cross-train as the Medical Records Specialist (MRS).

Job Qualifications
  • Education: High School graduate. Graduate of an accredited school of vocational nursing, preferred.
  • Licensure: Must have a current state license as a Licensed Vocational Nurse or Licensed Practical Nurse. A multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) is preferred. Current driver's license. If required to make patient visits, a current CPR card is necessary.
  • Experience: One year of experience in homecare (e.g., home health or hospice) is preferred.
  • Skills: Ability to establish and maintain effective working relationships with all staff segments. Good interpersonal skills. Nursing skills as defined by accepted standards if an LVN.
  • Transportation: Reliable transportation and valid auto liability insurance.
  • Environmental/Working Conditions: Performs duties in an office environment during agency hours, with potential exposure to toxic materials, toner, and weather conditions. Must have flexible schedule and extended hours.
Responsibilities
  • Complete workflow tasks and other assignments in the agency's electronic medical record.
  • Prioritize scheduling of new patient admissions in coordination with the Admin or Case Manager RN.
  • Coordinate care with nurses, patients, and interdisciplinary team members.
  • Provide effective communication to patients, staff, healthcare professionals, and referral sources.
  • Respond professionally and creatively to agency and patient needs.
  • Participate in agency-sponsored training and QAPI program.
  • If clinical, perform patient visits and complete documentation timely.
  • Promote agency philosophy and policies to ensure quality care.
  • Cross-train as Medical Records Specialist (MRS) as needed.
