Job summary

MaineGeneral Medical Center is seeking a Hospice Scheduler to coordinate patient visits, admissions, routine and specialized services, and to assign care team members based on availability and patient needs.

You will be the main contact for patients, families, and caregivers, document scheduling in EMR, collaborate with nurses, social workers, chaplains, and volunteers, and participate in team meetings to optimize care delivery.

Qualifications

  • Prior experience in healthcare scheduling, preferably in a hospice or palliative care setting.
  • Understanding of hospice care principles, practices, and regulations.
  • Excellent communication and interpersonal skills, with the ability to effectively communicate with patients, families, caregivers, and healthcare providers.
  • Detail-oriented with strong organizational skills and the ability to prioritize tasks in a fast-paced environment.
  • Proficiency in using electronic medical records and scheduling systems.
  • Compassionate and empathetic attitude towards patients and their families.
  • Ability to work collaboratively as part of a multidisciplinary team to achieve common goals.

Responsibilities

  • Assign appropriate care team members to patient cases based on availability, expertise, and patient preferences.
  • Communicate with patients, families, caregivers, and healthcare providers to coordinate schedules and provide updates on visit plans.
  • Ensure accurate and complete documentation of scheduling information in EMR and scheduling systems.
  • Collaborate with nurses, social workers, chaplains, and volunteers to ensure coordinated care for patients and families.
  • Participate in team meetings, case conferences, and other collaborative efforts to optimize patient care delivery.

Job Summary:As a Hospice Scheduler, you will play a vital role in coordinating and managing the scheduling of hospice services for our patients. Individual pay is determined based on factors such as experience, qualifications, skills, and market considerations. In addition to base pay, eligible team members may earn additional compensation through shift, weekend, holiday, on-call, and other applicable differentials.
