Job summary

VitalCaring is seeking a Patient Services Coordinator to orchestrate scheduling and care coordination in a fast-paced hospice setting. The role ensures patients receive timely, compassionate support by aligning clinicians, patients, and families with efficient workflows.

Ideal candidates bring strong organizational skills and 1+ year of scheduling/administrative experience in healthcare, with proficiency in EMR systems and clear, empathetic communication.

Qualifications

  • High school diploma or equivalent.
  • 1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred).
  • Strong organizational and multitasking skills.
  • Experience working with electronic medical records (EMR) or scheduling systems preferred.
  • Excellent communication and interpersonal skills.
  • Ability to prioritize, adapt, and problem-solve in real time.

Responsibilities

  • Schedule and coordinate patient visits across hospice clinicians to ensure timely, efficient care.
  • Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination.
  • Monitor schedules and follow up on missed, incomplete, or delayed visits.
  • Utilize EMR and reporting tools to track productivity, coverage, and scheduling optimization.
  • Partner with field teams and leadership to reduce missed visits and improve patient experience.
  • Communicate effectively with patients, families, clinicians, and referral partners.
  • Respond to changing patient needs and scheduling challenges with urgency and problem-solve.
  • Maintain accurate documentation and workflow completion within the EMR system.
  • Support quality initiatives, including participation in QAPI programs.
  • Promote a culture of compassion and professionalism in every interaction.
