Job summary

VitalCaring Group in Mississippi is seeking a Patient Services Coordinator (PSC) in Hospice to help coordinate care delivery, schedule patient visits, and connect clinicians, patients, and families with clarity and compassion.

You will use EMR systems, support daily operations, and contribute to QAPI and quality initiatives while maintaining documentation accuracy and patient-centered communication.

Qualifications

• High school diploma or equivalent required.
• 1+ year of administrative, scheduling, or office experience (healthcare or hospice preferred).
• Strong organizational and multitasking skills.
• Experience with EMR or scheduling systems preferred.
• Excellent communication and interpersonal skills.

Responsibilities

• Schedule and coordinate patient visits across hospice clinicians to ensure timely, efficient care.
• Collaborate with nurses, social workers, chaplains, and leadership to support daily care coordination.
• Monitor schedules and follow up on missed, incomplete, or delayed visits.
• Utilize EMR and reporting tools to track productivity, coverage, and scheduling optimization.
• Partner with field teams and leadership to reduce missed visits and improve patient experience.
• Communicate effectively with patients, families, clinicians, and referral partners.
• Respond to changing patient needs and scheduling challenges with urgency and problem-solving.
• Maintain accurate documentation and workflow completion within the EMR system.
• Support quality initiatives, including participation in QAPI programs.
• Promote a culture of compassion and professionalism in every interaction.
