Patient Resource Coordinator

Location: US-OR, Homebased EE Oregon

Responsibilities

• Coordinate the transition plan developed by Care Management Staff for patients discharging from Legacy facilities, including Unity services.
• Provide routine updates to relevant teams regarding community resource changes.
• Assist patients with flexible service items, such as setting up phones.
• Manage and provide Medicare patients with an Important Message from Medicare (IMM).
• Support Care Management teams primarily, with secondary support to other departments as needed.

Qualifications

• Education: BS/BA or equivalent experience required. Preferably in Behavioral Science for Unity.
• Experience: At least 1 year in case management, discharge planning, or care coordination in healthcare or community mental health settings.
• Skills: Strong organizational skills, knowledge of community resources, excellent communication, time management, computer literacy, data management, and ability to initiate and investigate referral pathways.

Compensation

$26.59 - $38.01 hourly wage.
