Role Description

Oak Street Health adopts a team-based approach to patient care. The Service Team provides high-touch assistance to our patients daily.

Patient Relations Managers (PRMs) are integral, acting as insurance experts across centers. They advocate for patients by helping navigate insurance options and bills, and serve as resources for the care and outreach teams regarding Medicare and eligibility. PRMs play a key role during Open Enrollment, ensuring patients are informed of their options.

Responsibilities:
  1. Learn Medicare details to assist patients with insurance navigation.
  2. Serve as an internal resource on insurance questions for clinic staff.
  3. Help patients navigate the healthcare system and resolve medical bills, advocating for them.
  4. Build trust with patients to advise on healthcare coverage.
  5. Manage new patient welcome visits and orientations.
  6. Assist with applying for public benefits like Public Aid, free phones, and prescription assistance.
  7. Support operational activities such as scheduling and billing.
  8. Provide exceptional customer service.
  9. Enhance patient engagement through events and tours.
  10. Perform other duties as assigned.

This role reports to the Manager of Care Services and offers significant advancement opportunities.

What we're looking for

Motivated, experienced Patient Relations Managers with:

  • A problem-solving mindset and positive attitude.
  • Desire to help build and improve our operating model.
  • Sales experience is a plus but not required.
  • Supportive attitude towards our older adult patient population.
  • Community health experience preferred.
  • Proficiency with PC skills.
  • Bachelor's degree or equivalent experience preferred.
  • Fluency in Spanish is a plus.
  • Experience in senior living or related fields is a plus.
  • Reliable transportation for working across centers.
  • US work authorization.

Anticipated Weekly Hours: 40

Time Type: Full time

Pay Range: $21.10 - $49.08 per hour, based on experience and other factors. Role may include bonuses or incentives.
