Job summary

Qualifications

• High school diploma or GED.
• At least 1 year of experience in healthcare setting preferred.
• Proficient in Microsoft Office products.

Responsibilities

• Schedules and confirms patient consultation and referral appointments with specialists.
• Documents referrals in the Electronic Health Record, including scheduling, notification attempts, and records collection.
• Tracks referrals to ensure documentation for continuity of care.
• Schedules follow-up appointments and coordinates with providers and patients to keep appointments.
• Communicates personal health information with external organizations to support continuity of care.
• Organizes and aggregates incoming patient health information for quality measures and attaches results to the EHR.
