Job Title: Patient Navigator Specialist


Location(s): Los Angeles, CA




Description:


  • The patient navigator for sickle cell disease will answer phone calls and messages from patients.

  • They will help navigate patients through the health system to ensure proper follow up and care is done. They will work with the patients to assist with any insurance barriers and challenges.

  • They will work directly with clinic staff, providers, and other individuals involved with the sickle cell program to make sure patients' needs are met.


Responsibilities:


  • The individual will complete the following tasks for the Sickle Cell Program in Med-Peds:

  • Answer patient phone calls and messages.

  • Direct and assist patients with their needs ensuring that they contact the correct department.

  • Create a phone log of all their phone calls.

  • Document all completed task for the day to ensure proper handoff.

  • Help assist and coordinate with other patient navigators to ensure work is being completed

  • Work with members of the sickle cell team and to complete other assigned tasks related to patient navigation based on the needs of the program.
