Summary

This is a part-time, 0.1 FTE position working in the new, Crisis Walk-In Center. Incumbent will work 10 hours per two-week pay period. The Crisis Walk-In Center is open Mon-Fri, 11am-8pm, and Sat-Sun 9am-2pm. Incumbent would work every other weekend. No holiday coverage required.

Position Summary

The patient navigator is a certified peer support specialist (CPSS) who works in the Crisis Walk-In Center. The navigator helps guide the patient through their experience seeking help for behavioral health conditions. The navigator collaborates with the care team, including nurses, counselors, and psychiatric providers, to assure that the patient has a follow-up care plan. The navigator is an advocate and helps the patient navigate the healthcare system and addresses social determinants of health and barriers to care.

Qualified individuals must have the ability, with or without reasonable accommodation, to perform the following duties:

  • Meet with patients presenting to the Crisis Walk-In Center, engage in recovery-oriented dialogue, and support the patient during their visit.
  • Collaborate with care team members to address the presenting needs of the patient.
  • Facilitate communication between the patient, family members, community supports, and healthcare providers.
  • Complete assessment of social determinants of health and, in conjunction with the team, develop a plan to address them.
  • Ensure continuity of care and linkage to community resources based on needs assessments.
  • Schedule patients for follow-up care as directed by the treatment team.
  • Document interventions and time spent with the patient in the electronic medical record.
  • Make outreach follow-up calls to patients served in the Crisis Walk-In Center.

The following duties are considered secondary to the primary duties listed above:

  • Maintain knowledge of registration and scheduling to support patient flow and follow-up care.
  • Other duties as assigned.
Responsibilities

Minimum Required Qualifications:

  • High school diploma or GED
  • Current certification as a Peer Support Specialist is required or certification within six months of placement date.
  • Three years of customer service or administrative support experience.

Preferred Qualifications:

  • Experience working in a healthcare or behavioral health care setting.
  • Experience working successfully on a multidisciplinary team.
