Job summary

Callen-Lorde Community Health Center in Manhattan, NY seeks a dedicated Patient Navigator to support HIV-positive patients. This grant-funded, full-time role focuses on biopsychosocial needs, case management, and adherence to primary care.

Responsibilities include field-based case management, coordination with care teams, and helping patients navigate resources to improve health outcomes. Evening hours are required, with a comprehensive benefits package.

Qualifications

  • College degree (BA/BS) in health/social services or related field preferred.
  • Experience in social services, case management and/or entitlements work.
  • Strong computer competency, including knowledge of Electronic Medical Records, Microsoft Office Programs, and internet research savvy.
  • Strong customer service orientation with an understanding of patient-led treatment.
  • Experience working with individuals with a spectrum of chronic illness conditions preferred.

  • Document patient interactions and progress notes to ensure continuity of care.
  • Contribute to a team-oriented environment, fostering a supportive and inclusive space for patients.

Work Schedule

This is a full‑time position with evening hours.
