Formación

• High School Diploma required; Bachelor's degree preferred.
• Relevant experience in health care, population health, care management, or related support role.
• Excellent verbal/written communication, interpersonal, problem-solving, and critical thinking skills.
• Proficiency in Microsoft Office (Excel, Access, Word).

Responsabilidades

• Supports care teams in improving quality performance and patient engagement.
• Coordinates follow-up appointments and preventive screenings with primary care staff.
• Improves documentation in the electronic health record and supports quality reporting.
• Assists in preventive services outreach to improve health maintenance adherence and close care gaps.
• Acts as a point of contact for patients and families to access primary care and preventive services.
