Qualifications

• Minimum 2 years of community outreach/engagement experience.
• High School Diploma or GED equivalence required.
• Proficiency in navigating community resources and advocating for patients.

Responsibilities

• Establishes relationships with patients and acts as primary contact for barriers to care and social determinants of health.
• Links patients to primary care and insurance when needed.
• Refers patients to community resources to address social determinants of health.
• Provides crisis intervention and connects patients to resources.
• Schedules appointments and makes warm referrals as needed.
• Documents and follows up on patient encounters and outcomes.
• Maintains referral lists and resources with the care coordination team.
• Assists in updating community resource databases and service providers.
