Cooper University Health Care in New Jersey is seeking a Patient Navigator to bridge gaps between clinicians and patients, coordinating non-clinical scheduling, pre-certification, and follow-up calls. Responsibilities

• Bridge gaps between clinician and patient to ensure continuity of care.
• Provide non-clinical scheduling support, pre-certification, follow-up calls, and record retrieval.
• Call new patients within 24 hours to check follow-up needs.
• Coordinate with CPN, Navigators, Social Work, Dietitians, Call Center, Providers and scheduling teams.
• Participate in daily rounds on inpatient oncology to identify coordination needs.
• Assist in Transition of Care from inpatient to outpatient settings.
• Connect patients to services such as social work or financial counseling.
• Track patient contacts, follow-ups, barriers, and outcomes.
• Coordinate customer services to ensure quality patient care.
• Participate in interdepartmental activities to support care quality.

Short Description

• Bridge the gap between clinician and patient, by providing individual support andassistance in navigating the health care system to provide continuity of care.
• Working with current RN Navigator disease team to provide non-clinical support regarding scheduling, pre cert process, follow up calls and record retrieval for timely appointments.
• Call new patients within 24 hours of visit to CCI to check in regarding follow up appointments and needs.
• Work with CPN, Navigators, Social Work, Dieticians, Call Center, Providers, Radiology scheduling, chemo scheduling, and phone triage to support patient's needs.
• Participate in daily multi-disciplinary rounding on inpatient oncology unit, and identify needs and opportunities for coordination of care, follow up and scheduling.
• Act as a member of the Transition of Care team assisting patients in transitioning from inpatient to outpatient areas without interruption in services and care.
• Assist in connecting patients to services for support (ie: social work, financial counseling).
• Tracking of all patient contacts, follow ups, barriers to care and outcomes.
• Ensures the delivery of patient care through the coordination of customer services
• Participates in interdepartmental and ancillary activities to ensure quality, cost-effectivepatient care.
• Responsible for all core competencies and Cooper standards of care.

Experience Required

• Experience working in a healthcare office setting, or community outreach and education required.
• Knowledge of health care, inpatient/outpatient settings, Oncology specific patient population
• Strong interpersonal skills; demonstrated ability to work well with people of various backgrounds, ethnicities, and life experiences.
• Ability to take direction, follows through with tasks, and work independently.
• Flexibility, optimism, and a cooperative working style.

Education Requirements

• High School Diploma or Equivalent; clinical training (i.e. MA, CNA, LPN) a plus
