Supports, educates and assists patients and their families, serves as a patient advocate. 25%Demonstrates effective teamwork as a member of the interdisciplinary care team, fostering and exemplifying a culture of respect. May participate in the direct care of patients during the Provider visit. 25%Responsible for the coordination of care, assists in securing prescriptions and supplies for proper care.
