Valley Health System is seeking a Patient Navigator for Luckow Pavilion on day shift. The role focuses on guiding patients through the health system, coordinating with departments to ensure timely access to services and collaborating with Nurse Navigator and care teams to address patient needs.

Ideal candidates will have an Associate degree (or higher) and Patient Navigator Certification; English fluency is required and PC proficiency is expected.

Qualifications

• Associates degree preferred; Patient Navigator Certification preferred or obtained within 1 year of employment.
• Basic understanding of medical terminology, health care systems and health care financing required.
• Proficiency with a personal computer is necessary.

Position SummaryThe Patient Navigator provides individualized service, navigating and supporting patients throughout the health system. He/she works in a dynamic health care environment with multiple departments to ensure timely screening, diagnosis, treatment and supportive services. S/he works in conjunction with the Nurse Navigator and other health care professionals to evaluate and address the needs of patients served to ensure timely access to services needed and optimize health and wellness.EducationAssociates Degree preferred, Patient Navigator Certification preferred, or obtained within 1 year of employmentExperienceBasic understanding of medical terminology, health care systems and health care financing required. Proficiency with a personal computer is necessary.
