Job summary

The UC Davis Health Home Care Services Department seeks a Non-Clinical Patient Navigator 3 to support patients, families, and partners through transitions to home care. This role prioritizes patient flow and timely processing of referrals, with potential weekend/holiday intake and phone coverage.

The position is 100% on-site in Rancho Cordova, with Mon-Fri 8am-5pm shifts. Strong knowledge of payer systems, HIPAA, and MS Office is expected, along with excellent communication and organizational

Qualifications

• Experience in a complex health system environment.
• Proficiency with MS Office and mainframe applications.
• Excellent written and verbal communication.
• Knowledge of payer systems (Medicare, Medi-Cal, managed care).
• Ability to follow workflows and maintain confidentiality.
