• Experience working within a complex health system environment.
• Thorough knowledge of computer systems and the ability to effectively utilize software applications including demonstrated proficiency in MS Office Suite and mainframe applications such as web-based search engines.
• Ability to communicate effectively both orally and in writing.
• Knowledge of all payer systems, procedures, and regulations including Medicare, Medi-Cal, managed care, and third party and experience obtaining authorizations.
• Ability to exercise initiative and judgment in applying established workflows.
• Ability to multitask and set priorities, organize work independently, set-up and maintain manual tracking systems for authorizations, follow-up, etc.
• Works effectively in an area with constant interruptions and traffic, including noise from telephones, staff conversations, and office equipment.
• Ability to recognize and maintain confidentiality of patient information and comply with all HIPAA laws.
• Ability to identify own professional strengths and weaknesses and to engage in activities designed to enhance effectiveness. Strong customer service, communication, and interpersonal-relationship skills to establish and maintain effective relationships with patients, families, team members, and hospital staff in a wide variety of positions and departments.
• Ability to be sensitive and effective with persons representing a broad spectrum of social, ethnic, and economic backgrounds. Ability to read, write, speak, and understand English language and to utilize medical interpretation services.

The Patient Navigator will provide non-clinical support, guidance, and assistance for patients, families, and other healthcare partners as they navigate through the complex healthcare needs of patients throughout the transition to home care. The Patient Navigator is responsible for ensuring patient flow; throughput is a priority by processing all referrals in a timely manner. This position may include intake/referral processing and phone coverage on weekends and/or holidays as needed.
