Job summary

Northeast Valley Health Corporation in North Hollywood, CA is seeking a Patient Navigator for the Enhanced Care Management (ECM) Program. The role guides patients to access and navigate health care and related social/financial services, performing health education, clerical and medical support in clinic and the community.

Reports to Program Manager I – Enhanced Care Management and Director of Quality. Requires CPR, English and Spanish fluency, and strong communication; includes confidentiality

Qualifications

• Fluency in Spanish (speaks, interpret, read, and write) and will complete the mandatory Medical Interpretation course.
• Ability to maintain absolute confidentiality about health care and other patient/client information.
• Ability to maintain a professional demeanor.
• If school-based site, willingness to submit fingerprints per California DOJ.
• Current California driver’s license, appropriate insurance, and driving record acceptable to NEVHC’s insurer.

The Patient Navigator – Enhance Care Management (ECM) Program is a community-based professional who serves as a guide to help individuals access and navigate the health care system, as well as related social and financial services, to improve health outcomes. The Patient Navigator performs a variety of health education, clerical, and medical support tasks both in the clinic and in the community. A primary responsibility of this role is to establish and maintain communication between patients, their families, providers, and the health care system to support coordinated care.

Reports to:

Program Manager I – Enhanced Care Management (ECMP) and Director of Quality of Quality Health Equity and Innovation

Qualifications:

• 1.High school diploma or GED.
• 2.Previous experience in a health care setting is preferred.

Additional Requirements for all levels:

• 3.Must maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy.
• 4.Good written and verbal communication skills.
• 5.Ability to be trained to perform a variety of tasks.

• 16.Current California driver’s license, appropriate insurance coverage, and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
