Job description

Under the direction of the Program Manager II for Pediatric Health Equity & Special Projects, the Patient Navigator II- Pediatric Initiatives serves as a dedicated guide and advocate for patients and their families and plays a crucial role in ensuring a timely completion of pediatric assessments. The Patient Navigator’s core responsibility is to optimize workflow efficiency by ensuring that the appropriate clinical staff are performing the assessments and following intervention protocols. The ultimate goal is to enhance patient experience and improve health outcomes for pediatric patients and their families.

Program Manager II – Pediatric Health Equity & Special Projects

Qualifications:

• 1. High School Diploma or General Education Development (GED) Certificate.
• 2. Bachelor’s degree in Health Education or related field OR 3 years of patient navigation experience or progressive experience is required.
• 3. Ability to communicate well orally and written, and give group presentations and one-on-one education.
• 4. Ability to work with diverse patient populations and at-risk clients.
• 5. Knowledge and experience working with health service benefits, social service programs, and/or mental health services.
• 6. Thorough working knowledge of business English, spelling, punctuation, and contemporary general office practices and procedures
• 7. Computer skills in Microsoft Office programs (Word, Excel, etc.), electronic health record systems, and database systems.
• 8. Effective verbal and written communication skills to communicate clearly and effectively with patients and others.
• 10. Ability to work effectively as a team player.
• 11. Ability to be flexible and work in a changing environment.
• 12. Sensitivity to the different cultures represented among members and staff.
• 13. Demonstrated ability to listen and communicate with others in a professional and caring manner including sensitivity with individuals from diverse cultures and lifestyles.
• 14. Demonstrated ability to set priorities for tasks to work effectively in spite of interruptions and under minimal supervision. Self-starter, reliable and dependable.
• 15. Demonstrated proficiency with the electronic health record database within three months of attending training session(s).
• 16. Fluency in English (speak, read and write) is required.
• 17. Fluency in Spanish (speak, translate, read and write) is required.
• 18. Ability to solve problems and make routine recommendations.
• 19. Ability to maintain absolute confidentiality about health care and other patient/client information.
• 20. Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job).
