The Patient Navigator coordinates and supports all aspects of an assigned clinical service under appropriate supervision. This role serves as a vital liaison between patients, providers, departments, and community partners to ensure seamless care coordination and an exceptional patient experience. The Patient Navigator helps patients access the care and resources they need by identifying and addressing barriers throughout their healthcare journey. Through education, advocacy, and coordination, this role promotes patient engagement and supports adherence to treatment plans.
As a Patient Navigator, you'll be a trusted resource for patients and families, helping them confidently navigate the healthcare system, access critical services, and receive coordinated, high-quality care.
