As a clinical resource with expertise in specialty care management, you are that critical connection and liaison for a patient population recovering from strokes, cancer, and long-term illnesses. Responsibilities
  • Coordinate written and verbal communication relating to the medical treatment plan between the patient, family, physician, physician staff, and other team members.
  • Communicate and coordinate the treatment plan to internal and external departments and agencies, establishing and maintaining the continuity of patient care.
  • Assist patients in understanding their diagnosis, treatment options, and the resources available.
  • Review the medical documentation to assist the physicians in obtaining complete and accurate documentation of their medical treatment plan.
  • Serve as an essential link between patients and all other care providers.
  • Follow patients throughout the care continuum collaborating with care management resources and facilitating appointments for consults and support services including social workers, PT, OT, ST, dietitians, physicians, financial resources, etc.
  • Collect data, track outcomes, and support strategic planning processes. Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance from screening to survivorship by planning, implementing, and evaluating patient’s care.
  • Assist patients and their family members in search of solutions, treatment-related problems, education to facilitate seamless care and support patients with difficult decisions.
  • Perform ongoing review of policies related to service provided where appropriate and update policies to enhance professional practice.
  • Serve as a resource for community educational events such as health fairs, screenings, symposiums, and lectures as well as staff education.
Qualifications
  • Current nursing license to practice in Louisiana and as defined by the Louisiana State Board of Nursing.
  • Amercian Heart Association BLS certification.
  • 3 years of professional nursing experience in specialty area.
