Patient Navigation Specialist Location: University Medical Center Department: Infectious Disease Clinic Shift: Day Shift Employment Type: Full-Time The Patient Navigation Specialist coordinates all aspects of assigned clinical services and serves as a key resource for patients throughout their healthcare journey. This role helps patients overcome barriers to care, facilitates access to clinical services, and supports patient engagement and adherence to treatment plans. The Patient Navigation Specialist acts as a liaison among patients, providers, healthcare teams, and community organizations to ensure seamless care coordination and an exceptional patient experience.

Key Responsibilities
Patient Navigation & Support
  • Assist patients in navigating healthcare services and accessing available resources.
  • Identify and help resolve barriers that may prevent patients from receiving care.
  • Provide guidance and education regarding clinical services and treatment processes.
  • Promote patient engagement and adherence to provider-directed plans of care.
Patient Outreach & Follow-Up
  • Contact patients who miss appointments and encourage re‑engagement in care.
  • Assist with scheduling and coordination of appointments and follow‑up services.
  • Maintain communication with patients to support continuity of care.
Care Coordination & Relationship Management
  • Collaborate with physicians, nurses, and interdisciplinary teams to achieve optimal patient outcomes.
  • Serve as a liaison between departments, patients, vendors, and community agencies.
  • Build and maintain relationships with internal and external referral partners.
  • Facilitate patient referrals and coordinate transitions between services.
Documentation & Data Management
  • Maintain accurate patient records and clinical documentation.
  • Upload and manage documentation within the Electronic Health Record (EHR) system.
  • Collect, track, and organize program‑specific data and reports.
  • Ensure confidentiality and compliance with healthcare regulations.
Community Outreach & Engagement
  • Coordinate community outreach programs and educational events.
  • Assist with planning support groups and patient education initiatives.
  • Identify guest speakers and educational resources for support group activities.
  • Promote available clinical services throughout the community.
Administrative Coordination
  • Schedule and coordinate departmental meetings, provider meetings, and special events.
  • Prepare materials, agendas, and communications for meetings as needed.
  • Assist leadership with special projects and operational initiatives.
Minimum Qualifications
Education and Experience
  • High School Diploma or GED required OR Minimum of two (2) years of relevant work experience
Preferred Qualifications
  • Experience working in healthcare, patient services, care coordination, or customer service.
  • Familiarity with community resources and referral management.
  • Experience supporting patient populations in a clinical setting.
  • Knowledge of healthcare terminology and medical office procedures.

  • Proficiency with EPIC Electronic Health Record (EHR) systems or similar healthcare software.
  • Strong computer and documentation skills.
