Mass General Brigham Community Physicians, Inc. is seeking a Patient Navigator to work on-site approximately four days per week at our Chestnut Hill practice.

This position requires the person in the role to work on-site approximately 4 times per week at our medical practice at 850 Boylston Street in Chestnut Hill, MA. The Patient Navigator is responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes
Qualifications Bachelor's Degree in related field of study required. Relevant experience may be accepted in lieu of a degree. 1+ years of experience in social work, community health, case management or related field required, preferably in a clinical setting.
Strong knowledge of healthcare resources, community services, and patient advocacy. Excellent communication and interpersonal skills. Ability to collaborate effectively with healthcare professionals across multiple disciplines and experiences. Strong organizational and time management skills. Familiarity with electronic health records and case management software.
Schedule and Work Model: Full-time, Monday through Friday, standard business hours. Hybrid work model: Requires up to 4 times per week in-office at 850 Boylston Street in Chestnut Hill, MA. Remote workday requires stable, quiet, secure, compliant workstation with MGB provided equipment.
