Mass General Brigham Community Physicians, Inc. is seeking a Patient Navigator to work with patients, families, and healthcare providers to streamline care and improve outcomes.
You will advocate for patients, coordinate appointments and follow-ups, connect patients with community resources, and educate them about conditions and self-care. The role is full-time with a hybrid schedule in Danvers, MA, with a pay range of $22.22–$31.71 per hour.

Responsible for working closely with patients, families, and healthcare providers to ensure a seamless and patient-centered care experience by assisting with navigating the complexities of the healthcare system, coordinating resources, and providing support to improve patient outcomes.
Qualifications: Bachelor's Degree in related field of study required; relevant experience can be considered in lieu of degree.
1+ years of patient facing, community health, medical office or other related experience strongly preferred.
Full-time, Monday through Friday, standard business hours. Hybrid work model: 1 day per week remote from home; 4 days per week in the office at 194 North Street in Danvers, MA. On remote workdays, employees must use a stable, secure, and compliant workstation in a quiet environment. Teams video is required and must be accessed using MGB-provided equipment.
