Job summary

Covenant Health in Knoxville, TN seeks a Consumer & Service Line Patient Navigator in the Call Center to coordinate patient-centered care. This full-time role (80 hours per pay period) supports patients, families, and physicians by expediting referrals, scheduling tests, and tracking follow-up.

Candidates should have at least two years of healthcare administration experience; high school diploma or GED accepted; an Associate’s degree is preferred.

Qualifications

Responsibilities

• Serves as primary liaison between hospital, clinic, specialty services, and Covenant Leadership.
• Advocates on behalf of the patient.
• Initiates referrals and ensures rapid access into the system.
• Schedules referrals, tests, and evaluations across offices.
• Gathers medical records and summarizes data for physician review.
• Addresses barriers to care and coordinates with support services.
• Supports post-discharge follow-up and care plan initiation.
