Job Summary: This position helps to navigate patients through the healthcare system, including providers' offices, hospitals, insurance and payment systems, patient-support organizations, and other components of the healthcare system. The Patient Navigator also helps to identify barriers preventing patients from receiving timely and high-quality healthcare treatment and works to find solutions to those barriers.
Responsibilities:
- Work with Health Center staff and community resources to become knowledgeable about available services that may assist patients.
- Assist in overcoming logistical barriers to scheduled diagnosis and treatment appointments.
- Conduct outreach to patients to emphasize the importance of preventive services based on age and health conditions.
- Help patients understand recommended follow-up for abnormal screening exams, treatment referrals, and preventive behaviors.
- Contact patients at risk for missing appointments and provide educational tools as needed.
- Facilitate access to resources for insurance coverage and sliding fee benefits.
- Ensure relevant information is available in patients' medical records during appointments.
- Facilitate linkages to follow-up services and coach patients to advocate for their own care.
- Build awareness of patient navigator services among the healthcare team and maintain communication to identify patients at risk for barriers to care.
- Communicate with providers about patients' unique needs, including cultural considerations.
- Perform other duties as assigned.
Qualifications/Requirements:
- Two years of healthcare experience required.
- Knowledge of healthcare systems, community resources, and HIPAA regulations preferred.
- High school diploma or equivalent required; Associate’s Degree preferred.
- N/A for licenses/certifications and other special requirements.
