Job summary

Pinnacle Fertility is seeking a compassionate Patient Navigator in Seattle, WA. You will be the primary contact for patients, guiding them through treatment plans and coordinating care with a multidisciplinary team.

Qualifications

  • High school diploma required; Bachelor’s preferred in healthcare or related field.
  • CMA certification preferred; healthcare knowledge and patient navigation experience valued.
  • Strong communication, organization, and empathy essential; EMR proficiency required.

  • Coordinate and schedule appointments, tests, and referrals for seamless care.

  • Oversee treatment coordination across departments and with finance, embryology, genetics.
  • Maintain accurate EMR documentation of interactions and care plans.
  • Provide emotional support and address barriers to care for patients and families.
  • Participate in team meetings, case conferences, and quality initiatives.
  • Educate patients on treatment protocols, self-care, and medication usage.
  • Track patient progress and follow up on missed appointments.
  • Advocate for patient needs and assist with after-hours call coverage.

About the Role

The Patient Navigatorposition is an exciting opportunity for compassionate, service-minded individuals who want to make a meaningful impact on patients' lives. In this role, you’ll serve as the primary point of contact for patients, guiding them through their fertility journey and working closely with a multidisciplinary team to ensure exceptional care and support.

We are seeking a Patient Navigator to join our dedicated team in Pinnacle Fertility - Washington in Seattle, WA. This is a full-time, onsite position, working Monday - Friday 7:30 AM - 4:30 PM.

Key Responsibilities
  • Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
  • Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
  • Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
  • Oversees the treatment coordination between departments for individual patient care plans working closely with financial, embryology, and genetics.
  • Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the electronic medical record (EMR) system.
  • Provide emotional support and resources to patients and their families, addressing any concerns or barriers to care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
  • Advocate for patients’ needs and help resolve any issues or barriers to care
  • Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.

Position Requirements
  • Education:High school diploma required. Bachelor’s degree in healthcare, social work, or a related field preferred; Certified Medical Assistant Certification (CMA) preferred.
  • Experience:Minimum of 2 years in customer service, patient navigation, case management, or a related healthcare role.
  • Skills:
    • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
    • Excellent communication, organizational, and problem-solving skills.
    • Proficiency in using electronic medical records (EMR) and healthcare software.
    • Empathy and a patient-focused approach, with the ability to work collaboratively in a multidisciplinary team.
    • Bilingual skills are a plus.
  • Hourly Rate: $23.00 - $38.00 (Final offers based on experience, skills, and qualifications).
