Job summary

Pinnacle Fertility in Downtown Portland, OR is seeking a dedicated Patient Navigator to guide fertility patients through their treatment journey. This on-site role works Monday–Friday, 8:00 AM–4:30 PM, with some holidays.

You will collaborate with a multidisciplinary team to educate patients, schedule tests, and coordinate care. Ideal candidates have at least 2 years in a clinical healthcare setting, strong EMR skills, and excellent communication and advocacy abilities.

Qualifications

  • High School Diploma required; Bachelor’s degree preferred.
  • Minimum 2 years in a clinical healthcare setting.
  • Must know healthcare systems, insurance processes, and patient advocacy; strong communication and problem-solving skills; EMR proficiency.

Responsibilities

  • Serve as the primary point of contact for patients, guiding them through their treatment plan and providing education on procedures, testing, and follow-up care.
  • Coordinate and schedule patient appointments, tests, and referrals to ensure a smooth continuum of care.
  • Assist patients with understanding their medical care, insurance coverage, and available financial resources.
  • Communicate with healthcare providers, clinical staff, and administrative teams to ensure seamless patient care coordination.
  • Maintain accurate and up-to-date patient records, including documentation of interactions and care plans in the EMR system.
  • Provide emotional support and resources to patients and their families, addressing concerns or barriers to care.
  • Participate in team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.
  • Educate patients on treatment protocols, self-care strategies, and medication usage.
  • Track patient progress, follow up on missed appointments, and ensure compliance with treatment plans.
  • Advocate for patients’ needs and help resolve any issues or barriers to care.
  • Assist with after-hours call coverage on a rotating schedule, ensuring patient needs are addressed appropriately.
  • Other duties and projects as assigned

We are seeking a Patient Navigator to join our dedicated team at Pinnacle Fertility - Oregon located in Downtown Portland, OR. This is a full-time, on-site position working from Monday - Friday from 8:00 AM - 4:30 PM schedule with some holidays required.

Key Responsibilities

COMPENSATION & BENEFITS
  • Hourly Rate: $26.00 - $30.00 per hour (Final offers based on experience, skills, and qualifications).
