Position Overview

I. Scope of Role:

The SRO Intervention Project works with homeless and formerly homeless people with HIV/AIDS and seeks to connect them to medical services and stabilized housing. The Care Coordination Redesign Program (CCR) employs a navigation and chronic care model to advocate for, communicate with, and identify resources for clients, supporting their independence in healthcare to improve health outcomes and quality of life.

Essential Duties & Responsibilities:
  • Provide direct, field-based services to enrolled consumers.
  • Support the implementation of a comprehensive Service Plan for each consumer.
  • Assist consumers with securing housing, medical care, nutrition, transportation, and benefits paperwork. Conduct social services and benefits re-assessments and follow-ups.
  • Visit consumers between clinic visits to reinforce treatment plans, provide appointment reminders, health education, coordinate transportation, and follow-up activities.
  • Accompany consumers to healthcare and social service appointments as needed.
  • Provide field-based Directly Observed Therapy (DOT) services, recording medication intake and side effects.
  • Offer feedback to healthcare team members based on field observations.
  • Record all services daily and document in AWARDS Case Management Software.
  • Collect and record Primary Care Status Measures data as assigned.
  • Participate in team meetings, case conferences, and trainings.
  • Engage in Quality Assurance/Quality Improvement activities.
  • Report to the Program Director in the absence of the CCR Supervisor.
Requirements
Qualifications
  • Minimum Associates Degree. Experience working with people living with AIDS, homeless individuals, or substance users encouraged. Effective written and oral communication skills.
Preferred Qualifications & Skills
  • Bi-lingual Spanish strongly preferred. Experience in outreach or case management preferred.
  • Basic computer skills (e.g., MS Office, Windows).
