Boston Children's Hospital is seeking a dedicated care coordination professional to collaborate with patients and families, review care plans, and facilitate referrals to clinic and community services. The role includes assisting with scheduling, insurance applications, and coordinating transportation to support plan adherence.
You will maintain relationships with community resources, document patient care, track outcomes, and participate in discharge planning.
Education:
High School Diploma / GED is required.
Bachelor's Degree is preferred. Human Services or other related fields of study preferred.
Experience:
A minimum of one year of closely-related experience is required.
Experience with the clinic population or work with community resources is preferred.
The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting.
Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
