Job summary

Boston Children's Hospital is seeking a Patient Navigator to educate and support patients and families, guiding them through the healthcare system and connecting them with BCH and community resources.

You will serve as a liaison for referring physicians, patients, and caregivers, review care plans, coordinate appointments, assist with insurance applications, and track outcomes to inform planning.

  • Documents patient care and analyzes data to support planning and partnerships.
  • Attends care planning meetings and disseminates training materials.

Position Summary/ Department Summary:

Provides education and emotional support to patients and families and helps guide them through the healthcare system. Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing supportive care services within BCH and in the community. Serves as a subject matter expert to Patient Navigators, Interns, Social Workers, and others on clinic and specialized services. Manages annual and ongoing projects such as clothing drives, Cradles to Crayons, and summer camps. May run workshops on topics such as housing and food insecurity. May help facilitate and assist with clinic-based groups requiring particular services.

Key Responsibilities:
  • Meets with/calls patients and families to review the implementation of ongoing care plans. Refers patients to services frequently used in a clinic and specialized services as needed. Explains hospital/community-based resources to patients and families, providing documentation.
  • Assesses patients for referrals to resources. Acts as a liaison with internal and community resources. May contact hospital and community resources to ensure access to services
  • May assist patients and families with scheduling multidisciplinary appointments. Reviews appointment schedule and appointment instructions. Coordinates logistics for plan adherence such as appointment reminders and arranging transportation.
  • Assists patients with applying for insurance. Provides insurance information to care teams to determine eligibility for services.
  • Initiates, develops, and maintains relationships for accessing community-based resources. Provides outreach and maintains positive working relationships with providers/administrative staff (including physicians, office staff, nurses, and social services staff). Reports outcomes of referrals to referral source.
  • Documents patient care and collects required data. Tracks outcomes and supports strategic planning processes. Develops and maintains data. Analyzes data and summarizes findings. Provides information to support community partnerships and grants. May recommend program or procedural changes to improve outcomes or efficiency.
  • Attends and participates in care planning meetings to prepare patients for discharge or continuing care. Disseminates training materials or ongoing education to larger program staff.

Minimum Qualifications
Education:

High School diploma/GED is required. Bachelor's Degree is preferred.

Experience:

A minimum of two years of experience including direct experience with the patient population and community resources is required.

The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting.
