Job summary

AtlantiCare is seeking a Health Coach to join the care team in a community setting. You will work with patients with complex medical and social needs, coaching via telephone, home visits, and in-person appointments to support navigating the health system and linking to community resources.

The role involves collaboration with the care team to implement care plans, travel within the community, and providing scheduling support as needed.

Qualifications

• High School Diploma required; Associates preferred.
• AHA BLS/HCP certification required within six months of hire or transfer.
• Valid NJ Driver’s License required.

Responsibilities

• Engages patients by telephonic coaching, home visits, hospital visits, and accompanying patients to provider appointments.
• Facilitates adherence to treatment plan by identifying and resolving care gaps.
• Provides appointment scheduling and clerical support as needed and assigned by program manager.
