Job summary

Saint Peter's Health Care System is seeking a Patient Flow Coordinator Per Diem in Clinical Operations. The role will identify and prioritize admissions, assign patients to appropriate units, and communicate bed assignments to receiving areas.

You will continually assess flow and notify the right people of delays or issues in movement. The position requires three years of experience with accuracy and attention to detail, strong verbal and written communication, and excellent customer service

Responsibilities

• Identify and prioritize patient admissions through hospital records and various departments.
• Assign patients to floor, bed type, and status per admission criteria or physician orders.
• Notify receiving units of admission details and bed assignments.
• Monitor patient flow and alert appropriate staff to delays or flow issues.
• Keep directors, nurse managers, and directors informed of clinical and operational issues such as discharges and admissions.
• Coordinate with Care Coordination on scheduled ambulance transfers and discharges.

Patient Flow Coordinator Per Diem

Clinical Operations

The Patient Flow Coordinator Per Diem will:

• Identify and prioritize patient admissions through admission records and within the hospital via recovery room, emergency department, etc., records.
• Assign patients to appropriate floor, bed type, and status based on criteria identified upon admission or physician orders.
• Notify the receiving units of the patient admission particulars and bed assignment.
• Continually assess the patient flow situations and notifying the appropriate persons when there are flow issues and delays in movement.
• Assure that director, nurse managers and directors are kept aware of related clinical and operational issues (discharges, admissions).
• Collaborate with Care Coordination staff on scheduled ambulance transfers and discharges.

Requirements:

• Three (3) years successful prior experience in a position requiring accuracy and attention to detail.
• Medical Terminology a plus but not required.
• Excellent Customer Service skills, including tact, diplomacy and sensitivity.
• Strong verbal and written communication skills.
• Demonstrated ability to work in a culturally diverse setting with diverse contacts, both internally and externally.
• Able to perform general system computer skills; familiarity with MS Office applications

Salary Range:18.47 - 28.61 USD

We offer competitive base rates that are determined by many factors, including job-related work experience, internal equity, and industry-specific market data. In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials.

The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
