Job summary

University Health seeks a Senior Social Work Manager to lead a multidisciplinary care team in an inpatient setting in San Antonio, TX. You will oversee care coordination, ensure payer requirements are met, and promote patient-centered planning from admission through discharge.

Ideal candidates hold a Master’s in Social Work, have 3+ years of hospital inpatient experience, and at least 1 year in management or senior social work roles. CCM or ANCC certification is desirable.

Qualifications

• Master’s degree in social work is required.
• Minimum of one year recent management or senior social worker experience.
• 3 years of hospital (inpatient) experience.
• 2 years inpatient case management or utilization management preferred.

Responsibilities

• Manages assigned staff to insure a smooth patient flow from preadmission to discharge.
• Works with a multi-disciplinary team to ensure a patient-centric care plan.
• Facilitates delivery of services, evaluates effectiveness, tracks outcomes.
• Functions as the manager of the team.
