Job summary

UT Health San Antonio is seeking a Case Manager to coordinate activities that promote quality outcomes, patient throughput, and discharge planning. You will identify barriers to throughput and minimize delays in discharge planning while collaborating with physicians, nurses, and other healthcare professionals.

You will advocate for patients and their families, coordinate with community resources, and ensure accurate documentation of assessments and care plans.

Qualifications

• Bachelor's in Nursing or Master's in Social Work required.
• LMSW or RN license required upon hire.
• Minimum 2 years hospital or acute care experience.

Responsibilities

• Coordinate discharge planning and care transitions.
• Collaborate with physicians, nurses, and other professionals to ensure coordinated patient care.
• Identify barriers to patient throughput and minimize delays in discharge planning.
• Develop and implement care plans and follow-up arrangements.
• Maintain accurate patient records and documentation.
