Job summary

Bay Area Hospital is seeking a Manager of Case Management & Social Services to lead case management, social services, discharge planning and transitions of care. This role ensures patient-centered coordination, regulatory compliance and efficient movement across the continuum.

The Manager partners with physicians, nursing leadership, Quality, Revenue Cycle and post‑acute providers to optimize outcomes and resource use while guiding staff and meeting organizational goals.

Qualifications

• Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or related field is required.
• Master’s degree preferred.
• Current, unrestricted professional licensure related to the discipline preferred (e.g., RN in Oregon for nursing).
• Certification in Case Management (CCM/ACM) preferred.

Responsibilities

• Provide operational and clinical leadership for Case Management, Social Services, discharge planning and care coordination.
• Oversee utilization management and transitions of care to ensure timely, patient-centered movement.
• Collaborate with physicians, nursing leadership, Quality, Compliance and post-acute partners to improve outcomes.
• Develop department budgets, staffing plans and performance improvement initiatives.
• Ensure compliance with CMS Conditions of Participation and payer requirements.
