Qualifications

• 1 to 3 years of experience in a Care Team or Patient Navigation Role.
• Healthcare experience preferred.

• Self-motivated with the ability to manage time appropriately.
• Experience with MS Office products (Outlook, Word, Excel, etc.).
• Minimum associate level education preferred. (High school diploma or equivalent is required.)
• 1 year of medical knowledge preferred.
• Dependable transportation

Responsibilities

• Enhance the overall patient experience and ensure patients receive the necessary support and resources.
• Accompany patients to their benefit renewal appointments to advocate for the patient and the program.
• Visit patient homes and update patient care.
• Effectively communicate and provide information to patients, caregivers, and physicians.
• Understand HIPAA and maintain confidentiality.
• Display a warm and friendly attitude.
