Foundation Medical Partners’ Amherst Family Practice is seeking a Patient Service Representative to create a positive patient experience from arrival through care coordination. You will support front office tasks and ensure correct patient information in the EMR while providing outstanding customer service.

The role emphasizes teamwork, adaptability across locations, and a commitment to patient-centered care in a fast-paced healthcare environment.

Qualifications

• High School diploma or equivalent is required.
• Experience as a Patient Service Representative or in a medical office is preferred, but not required.
• Experience in healthcare customer service is preferred.
• Familiarity with Epic EMR and Microsoft Office is expected.

Responsibilities

• Greet patients and visitors in a friendly, professional manner.
• Check patients in and out for appointments.
• Verify and update patient demographics, registration, and insurance information within the electronic medical record.
• Schedule appointments and assist in coordinating patient care needs.
• Respond to patient phone calls and inquiries in a timely and courteous manner.
• Communicate effectively with providers, clinical staff, and outside offices to support continuity of care.
• Maintain accurate patient records and ensure confidentiality of information.
• Deliver an exceptional patient experience while building meaningful connections within the community.
• Support office operations and assist at other practice locations as needed.
