Job summary

Giving Home Health Care is seeking a Patient Coordinator to act as the liaison among patients, caregivers, and internal teams. You will help ensure clear communication, support coordination of care, and promote a positive patient experience across multiple departments.

The role requires travel 3–4 days per week locally, with mileage reimbursement. A flexible schedule may be needed outside normal hours; no weekend work is required.

Qualifications

• 2–5 years of customer service and/or case management experience in healthcare or home health preferred.
• High school diploma or equivalent.
• Valid New Mexico driver’s license, current auto insurance, and dependable personal vehicle for patient visits.
• Ability to travel 3–4 days per week.
• Computer literacy — comfortable using tablets and laptops; proficient in MS Office (Outlook, Word, Excel, PowerPoint).
• Strong written and verbal communication, interpersonal, and presentation skills.

Responsibilities

• Serve as liaison between Giving Home Health Care, patients, caregivers, and internal departments to ensure clear communication.
• Support patient needs and coordinate care, ensuring services are delivered efficiently and compassionately.
• Promote patient satisfaction and continuity of care while building strong relationships with patients.
